All information must be supplied. Do not leave anything incomplete.  If you are not required by law to file taxes each year, upload the same document you used for the proof of income section. You can take a picture of your SSI or other document you receive.

If you worked and are required to file taxes, you must upload those taxes or your application is incomplete. ONLY senior citizens and others are not required to file.

Do you understand that it will be your responsibility to pick up any donated furniture when your appointment time is given? We will work with you on the time/day as much as we can when your caseworker sets it up. We do not have the ability to deliver.:
A caseworker from United Way of Southwest Louisiana will follow up on this request in the next 10-14 days if you qualify. Do not call the office. You will not receive a call on the same day you apply. Not answering phone calls or emails will result in an incomplete application.
